1、杭州市江干区人民医院“助护”工程培训方案一、培训目的组织专业的护理人员上门为生活自理能力重度依赖或完全依赖的失能老人家庭中的护理人员，开展家庭护理和家庭康复知识技能培训，帮助老年人及其家属掌握基本的护理和康复知识技能。二、指导老师资质要求工作10年以上，具有丰富临床经验和专业技能的护师、主管护师、副主任护师。海潮院区具体名单如下: 郑霞静、柳红菊、支囯娣、麻巧玲、龚文利、郑雅红、郑雪文、郑芳、董春英、郑丽君、杭伟亚、孙连儿、俞小玲、徐英美、孙文英、李可可、马慧飞、柴婧、赵爱农、雷红英、葛冬兰、陈爱玲、金岙连、袁亚琴、郑怀瑾、沈建红、郭黎红、朱志萍、洪娜、刘仙花笕桥院区另定三、培训要求1、掌握居

7、的方法）13、管饲的护理14、导尿管的护理15、造瘘管的护理16、人工肛门的护理17、肢体的功能锻炼18、窒息的初步急救19、心跳骤停的初步急救20、呼叫“120”的注意事项1一、日常生活照护的注意事项老人在卫生、饮食、大小便、起居等日常生活方面自理能力差，需要家属督促或协助。安排病人合理而有规律的生活，要求他们按时起床和就寝，进餐，使之生活接近正常规律，保证足够的休息和睡眠时间。 维持良好的个人卫生习惯，可减少感染的机会。个人卫生包括皮肤、头发、指甲、口腔等的卫生，要求早晚刷牙、洗脸，勤剪指甲，定期洗头、洗澡，勤换内衣、被褥。给予卫生指导，采取措施制止不卫生行为，如随地大小便、捡地上东西吃等

8、。根据天气变化及时添减衣被，居室常开窗换气，被褥常晒太阳。长期卧床者要定期翻身、拍背，预防褥疮等护理。对病情较重的病人，要协助料理生活，照顾营养饮食、衣着冷暖和个人卫生。 白天尽量进行一些有益于身心健康活动，如养花、养鱼、画画、散步、太极拳、编织等，另外，也可读报、听广播、选择性看一些文娱性电视（忌看恐怖、惊险及伤感的节目），使病人充分感受生活的乐趣，保持轻松、愉快的心情。 病人往往有睡眠障碍，要为病人创造入睡条件，周围环境要安静，入睡前用温水洗脚，不要进行刺激性谈话或观看刺激性电视等。不要给老人饮酒、吸烟、喝浓茶、咖啡，以免影响睡眠质量。对严重失眠者可给予药物辅助入睡，夜间不要病人单独居住，

9、以免发生意外。二、老人的活动原则（1）因人而异，选择适宜：应根据自己的身体状况、所具备的条件，选择适合自己的运动种类、时间、地点。一般而言，运动时间以每日12次、每次30分钟为宜，每日运动的总时间不超过2小时；运动的场地最好选择在空气新鲜、环境清净、地面平坦的地方；运动的强度应根据老年人运动后心率而定，其计算方法为：一般老年人运动后最宜心率（次/分）=170年龄；身体健壮的老年人可采用运动后最高心率（次/分） =180年龄。（2）循序渐进，持之以恒：活动或运动的强度应由小到大、逐渐增加，并长期坚持。（3）自我监护，确保安全：在活动或锻炼过程中，一定要注意自我感觉。当出现不适感觉时，应立即停止活

10、动；出现严重不适感觉时，应及时就医。三、预防跌倒的方法1、加强照明，卧室和走道留夜灯，室内光线均匀、柔和；2、尽量减少台阶、门槛，在经常活动的地方，不堆放杂物；3、固定地毯，卫生间加防滑垫，保持居室地面干燥；4、室内的家具尤其是床、桌、椅的高度和摆放位置要合理，电线应收好固定，不要经常更换家具位置；保持地面平坦无障物；5、日常用品不宜放得过高或过低，易于取放；6、浴室和坐便器旁加装扶手；7、避免穿高跟鞋、软底鞋，少穿拖鞋，鞋子的大小要刚好，鞋底防滑。避免穿长短不合适的衣裤；8、体能虚弱、反应迟钝，有体位性低血压，服用安眠药以及用降压药的老人，夜间尽量不去厕所，如夜尿较频，可在床边准备好夜间所需

11、物品和便器。四、老年人衣着与饮食及生活规律衣着：老年人衣着服饰的选择，应以暖、轻、软、宽大、简单为原则。老年人的衣服要求宽大、轻软、合体，穿起来感觉舒适，同时衣服样式要简单、穿脱方便，不要穿套头衣服，纽扣多的衣服也不宜多，宜穿对襟服装。 2饮食：1、要保证充足的营养，营养的平衡，适当的摄入热量，还有维生素，给老年人补钙，饮食应该吃一些易于消化的东西，老年人的消化系统功能都变弱，食物应该更加细，软，这样易于老年人的消化。2食物的温度应该适宜，老年人的消化道比较敏感，饮食应适当的偏热，除了解除疲劳还可以使身体更加温暖，要让老年人养成良好的饮食习惯，避免暴饮暴食，或者过饥过饱。3早晨要吃好，中午要吃
