Certificate of Vessels NationalityRegistration Items: Items 1) to 10) are the same as that in Certificate of Vessels Registration 11) Name and Residence of Operator 12) This certificate is valid up to 3. Cargo Ship Safety Construction CertificateDistinctive Number or LettersDeadweight IMO NumberChina Classification Society certificates:1) That the ship has been surveyed in accordance with the requirements of regulations /10 of the Convention.2) That the survey showed that the condition of the structure,machinery and equipment as defined in the above regulation was satisfactory and the ship complied with the relevant requirements of Chapter -1 and - 2 of the Convention.3) That the last two inspections of the outside of the ships bottom took place on and 4) That an Exemption Certificate has/has not issued.4. Cargo Ship Safety Equipment Certificate China Classification Society certificates:1)That the ship has been surveyed in accordance with the requirements of regulations /8 of the Convention.2)That the survey showed that:(1) The ship complied with the requirements of the Convention as regards fire safety system and appliances and fire control plans;(2) The life-saving appliances and the equipment of the lifeboats,liferafts and rescue boat were provided in accordance with the requirements of the Convention;(3) The ship was provided with a line-throwing appliance and radio installations used in life-saving appliances in accordance with the requirements of the Convention; (4) The ship complied with the requirements of the Convention as regards ship-borne navigational equipment,means of embarkation for pilots and nautical publications;(5) The ship was provided with lights,shapes,means of making sound signals and distress signals,in accordance with the requirements of the Convention and the International Regulations for Preventing Collision at Sea in force; (6) In all other respects the ship complied with the relevant requirements of the Convention.3) That the ship operates in accordance with regulation /26.1.1.1 within the limits of the trade area.4) That an Exemption Certificate has/has not issued. This certificate is valid until subject to the annual and periodical surveys in accordance with regulation /8 of the Convention.5. Cargo Ship Safety Radio Certificate China Classification Society certificates:1) That the ship has been surveyed in accordance with the requirements of regulations /9 of the Convention.2) That the survey showed that:(1)The ship complied with the requirements of the Convention as regards radio installations;(2) The functioning of the radio installations used in life-saving appliances complied with the requirements of the Convention.(3) That an Exemption Certificate has/has not issued. This certificate is valid until subject to the periodical surveys in accordance with regulation /9 of the Convention.6. Minimum Safety Manning Certificate This is to certify that the ship will be considered to be safely manned provided that she has not less than numbers and grades of personnel shown in this Certificate. Note:1) Sailor shall hold the Certificate of Competency for rating forming part of a navigational watch. Motorman shall hold the Certificate of Competency for rating forming part of a engineering watch.2) The number of personnel holding the Certificate for Proficiency in Survival Craft and Rescue Boat shall not be less than the number of such craft and boats on board in case of passenger ship.3) The numbers and grades of personnel given in this Certificate are the minimum manning requirement necessary for the ships safe operation and do not preclude the carriage of additional personnel for the purpose of ships maintenance,livelihood service,cargo handling,labor protection,etc. However,the total number of the personnel on board can not exceed the limitation of the life-saving appliances.7. Ship Safety Navigation Certificate This is to certify that the ship has been specially surveyed in accordance with Regulations and Rules of this Society and found in a fit and efficient condition.The ship operates within the limits of the trade area Summer freeboard to center of Load Line disc. mmThis Certificate is issued under the authority of This Certificate is valid until 1) Anchoring Equipment: (1) Number,type and weight of anchors: (2) Length of chain cables and dia. of links: 2) Fire-fighting Appliances(1) fire main system(2) CO2 or halon or other systemFire pump: type; number; capacity; pressure headFire extingushant: kind;weight; practical: Spaces protected fire detection and fire alarm system Other fire appliances3) Life-saving Appliances The life-saving appliances provided for a total number of persons and no more.(1) Type,number of lifeboat,No. of persons certified to accommodate (2) Type, number of liferafts,No. of persons certified to accommodate (3) Number of buoyant apparatus,No. of persons certified to accommodate (4) Number of lifebuoys Number of lifejackets (5) Number and type of line throwing appliance and distress signals 4) Signal Equipment The ship complied with the provisions of the Signal Equipment of Sea-going Ships of the Government of the Flag State.5) Radio Equipment The ship complied with the provisions of the Radio Equipment of Sea-going Ships of the Government of the Flag State.8. International Load Line CertificateChina Classification Society Certifies: 1) That the ship has been surveyed in accordance with the requirements of Article 14 of the Convention. 2) That the survey showed that the freeboards have been assigned and load lines shown above have been marked in accordance with the Convention. 3) This certificate is valid until subject to annual surveys in accordance with Article 14 (1) (c) of the Convention.9. Ships Sanitary Certificate In accordance with the Frontier Health and Quarantine Law of the Peoples Republic of China and its specific rules and other health regulations concerned. The sanitary condition of the above ship is considered satisfactory as a result of inspection. The certificate is hereby granted.10. Deratting Certificate / Deratting Exemption Certificate This certificate records the inspection and deratting / deratting exemption at this port and on the above date of the ship under the flag of net tonnage . At the time of inspection / deratting,the holds were laden with tons of cargo.Recommendation: In the case of exemption,state the measures taken for maintaining the ship in such a condition that it is free of rodents and the plague vector. Note: Deratting measures: (1) by fumigation: fumigant(2) by catching,trapping or poisoning11. International Oil Pollution Prevention Certificate China Classification Society certificates: 1) That the ship has been surveyed in accordance with the Regulation 4 of Article of the Convention. 2) That the survey showed that the structure,equipment,system,fittings,arrangement and material of the ship and the condition thereof are in all respects satisfactory and that the ship complies with the applicable requirements of Annex of the Convention. 3) This certificate is valid until subject to the periodical surveys in accordance with Regulation 4 of Annex of the Convention.12. Ship Gas Free CertificateThis is to certify that following accessible compartments or spaces of the said ship have been inspected and tested. The above-mentioned compartments and spaces have been found free from combustible gas. Explanations: 1) This Certificate does not include all kinds of piping,pump and other equipment. 2) This Certificate would not remain valid,should the combustible gases or inflammable materials enter the tested spaces due to the change of ballast,opening of piping and valves or other causes.3) Before the hot-work is commenced and while it is proceeding the work areas including the heat influenced adjacent spaces are to be retested in due time by competent persons of the working unit. Note: the Compartments or spaces are referred to: (1) Cargo tanks ( port/center/starboard) (2) Cargo oil pump rooms (3) Ballast tanks (4) Slop tanks (5) Cofferdams(6) Other compartments or spaces13. Sewage Pollution Prevention Certificate China Classification Society certificates:1) That the ship is equipped with a sewage treatment plant / comminuter / holding tank and a discharge pipeline in accordance with Regulation 3 (1) (a) (i )to () of Annex of the Convention. A pipeline for the discharge of sewage to a reception facility,fitted with a standard shore connection.2) That the ship has been surveyed in accordance with Regulation 3 of Annex of the Convention and the survey shows that the equipment of the ship and the condition thereof are in all respects satisfactory and the ship complies with the applicable requirements of Annex of the Convention.3) This certificate is valid until . Under the provisions of Regulation 7(2) and (4) of Annex of the Convention,the validity of this Certificate is extended until .14. Passenger Ship Safety Certificate China Classification Society certifies: 1) That the ship has been surveyed in accordance with the requirements of regulation / 7 of the Convention. 2) That the survey showed that: (1) the ship complied with the requirements of the Convention as regards:a. the structure,main and auxiliary machinery,boilers and other pressure vessels;b. the watertight subdivision arrangements and details;c. the following subdivision load lines;(2) the ship complied with the requirements of the Convention as regards structural fire protection,fire safety systems and appliance and fire control plan;(3) the life-saving appliances and the equipment of the lifeboats, liferafts and rescue boats were provided in accordance with the requirements of the Convention;(4) the ship was provided with a line-throwing appliance and radio installations used in life-saving appliances in accordance with the Convention;(5) the ship complied with the requirements of the Convention as regards radio installations;(6) functioning of the radio installations used in life-saving appliances applicable complied with the requirements of the Convention;(6) the ship complied with the requirements of the Convention as regards ship-borne navigational equipment,means of embarkation for pilot and nautical publications;(7) the ship was provided with lights,shapes,means of making sound signals and distress signals in accordance with the requirements of the Convention and the International Regulation for Prevention Collisions at Sea in force;(8) in all other respects the ship complied with the relevant requirements of the Convention. 3) That an Exemption Certificate has/has not issued. This certificate is valid until .15. To Harbor Superintendency Administration of the Peoples Republic of China.Standard Orders on Board Vessel1. Standard Wheel Orders1) All wheel orders given should be repeated by the helmsman and the officer of the watch should ensure that they are carried out correctly and immediately. All wheel orders should be held until countermanded. The helmsman should report immediately if the vessel does not answer the wheel.2) When there is concern that the helmsman is inattentive he should be questioned : “What is your course?” And he should respond:“My course degrees.”This is a reminder to the helmsman to mind your helm.3) Orders and Meanings(1) Midships: Rudder to be held in the fore and aft position.(2) Port five/ten/fifteen/twenty/twenty-five: 5/10/15/20/25of port rudder to be held.(3) Hard-a-port: Rudder to be held fully over to port. (4) Starboard five/ten/fifteen/twenty/twentyfive: 5/10/15/20/25of starboard rudder to be held. (5) Hard-a-starboard: Rudder to be held fully over to starboard.(6) Ease to five/ten/fifteen/twenty: Reduce amount of rudder to 5/10/15/20and hold.(7) Steady: Reduce swing as rapidly as possible.(8) Steady as she goes: Steer a steady course on the compass heading indicated at the time of the order. The helmsman is to repeat the order and call out the compass heading on receiving the order. When the ship is steady on that heading,the helmsman is to call out “Steady on ”(9) Keep buoy/mark/beacon/on port side/ starboard side.(10) Report if she does not answer wheel.(11) Finished with wheel.4) When the officer of the watch requires a course to be steered by compass,the direction in which she/he wants the wheel turned should be stated followed by each numeral being said separately,including zero,for example: “Port,steer one eight two” ; “Starboard ,steer zero eight two”.5) The person giving the order should acknowledge the helmsmans reply.If it is desired to steer on a selected mark the helmsman should be ordered to: “Steer onbuoy/mark/beacon.”The person giving the order should acknowledge the helmsmans reply.2. Standard Engine Orders 1) Any engine order given should be repeated by the person operating the bridge telegraph(s) and the officer of the watch should ensure the order is carried out correctly and immediately. 2) Orders and meanings(1) Full ahead: Maximum manoeuvring engine revolutions for ahead propulsion(2) Half ahead: Revolutions as indicated in ships orders(3) Slow ahead: Revolutions as indicated in ships orders(4) Dead slow ahead: Revolutions as indicated in ships orders(5) Stop engine(s): No engine revolutions(6) Dead slow astern: Revolutions as indicated in ships orders(7) Slow astern: Revolutions as indicated in ships orders(8) Half astern: Revolutions as indicated in ships orders(9) Full astern: Revolutions as indicated in ships orders(10)Emergency full ahead/astern(11) Stand by engine: Engine-room personnel fully ready to manoeuvre and bridge manned to relay engine orders.(12)Finished with engine(s): Movement of engine(s) no longer required.3) In vessels fitted with twin propellers,the word “both” should be added to all orders affecting both shafts,e.g. “Full ahead both”,and “ Slow astern both”,except that the words “Stop all engines” should be used,when appropriate. When required to manoeuvre twin propellers independently,this should be indicated,i.e. “full ahead starboard”,“half astern port ”,etc.4) Where bow thrusters are used,the following orders are used: (1) Bow thrust full/half to port/ starboard side: Ships head to move to port/ starboard with power as specified.(2) Stern thrust full/half to port/ starboard side: Ships stern to move to port/ starboard with power as specified.(3) Bow (stern) thrust stop5) For vessels with variable pitch propellers,the meaning of the order would include the combination of
