1、1 定义Definition1.1药品不良事件是指药物治疗过程中所发生的任何不幸的医疗卫生事件，而这种事件不一定与药物治疗有因果关系。药品不良事件包括二个要素：一是不良事件的发生是由上市药品引起的，二是产生的结果对人体有害。它包括用药错误、临界错误和药品不良反应。Adverseeventofmedicationreferstoanyunfortunatemedicaleventoccurredinthecourseofpharmaceuticaltreatment,butsucheventsnotnecessarilyhavecausalrelationshipwithpharmaceutic
2、altreatment.Itincludestwoelements:1,theadverseeventiscausedbymedicationsappearedonthemarket;2,theresultingconsequenceisharmfultohumanbody.Itincludesmedicationerror,nearmissandadversedrugreaction.1.2用药错误是指药品使用过程中出现的任何可防范的、可能引起或己经造成不恰当地应用药品或伤害的事件。临界错误是用药错误的一部分，但在药物用于患者之前被中途拦截，属于未遂事件。Medicationerrorref

4、medicationwhichappearedwhenstandarddrugswereusedinnormalmethodsanddoses.2 分级Grading按照患者受损害程度，可分为5级。5gradescanbeclassifiedbasedondegreeofharmtothepatient.2.1临界错误：未到达患者，近似错误。nearmiss：notreachingthepatient,approximateerror.2.2无伤害：已到达患者，但未导致患者伤害。noharm：reachedthepatientbutnoharmtothepatient.2.3轻度：己到达患者，
5、导致患者轻度伤害，不需要进行治疗。mildharm：reachedthepatient,resultedinmildharm,noneedoftreatment.2.4中度：巳到达患者，导致患者中度伤害，需要住院或延长治疗时间。mediumharm：reachedthepatient,resultedinmediumharmwhichrequireshospitalizationorprolongedtreatment.2.5重度：巳到达患者，导致抢救、患者器官重大损害或死亡。seriousharm：reachedthepatient,resultedinrescueoflife,severe
6、damageoforgansordeathofthepatient.3 报告流程Reportingprocess3.1护士、医生或临床药师等一旦发现临界错误，应立即纠正，同时在5个工作日内填报不良事件报告表。Oncenearmissisdiscovered,thenurse,doctororclinicalpharmacistshallrectifyitatonceandfillinReportofadverseeventvwithin5workingdays.3.2护士、医生或临床药师等一旦发现因用药错误引起的药品不良事件，应立即报告病人的主管医生，同时在24小时内填报不良事件报告表，并通告
7、医务处及药剂科，预防同类事件在本院重复发生，保障患者用药安全。Onceadverseeventduetomedicationerrorisdiscovered,thenurse,doctororclinicalpharmacistshallnotifytheattendingdoctorofthepatient,fillinuReportofadverseeventvwithin24hoursandreporttodeptofmedicaladministrationandpharmacytopreventreoccurrenceofsimilareventsandguaranteepatie
8、ntsafety.3.3护士、医生或临床药师等一旦发现药品不良反应，应立即报告病人的主管医生，按照相关要求填报药品不良反应报告表。Onceadversedrugreactionisfound,thenurse,doctororclinicalpharmacistshallnotifytheattendingdoctorofthepatientandfillinuReportofadversedrugreactionv.3.4临床医生在病历上记录因药品不良事件而采取的措施。Clinicianshallwritemeasurestakentodealwiththeadverseeventdueto
9、medicationincasehistory.3.5临床医师与药师跟踪/随访药品不良事件，记录治疗及预后情况。Clinicianandpharmacistshalltrace/followupadverseeventofmedication,recordtreatmentandprognosis.4处理反馈Feedback4.1医院采取无惩罚性药品不良事件报告制度，鼓励各级医务人员踊跃呈报药品不良事件。Thehospitaladoptsareportingsystemofadversemedicationeventwithoutpunishmentandencouragesmedicalst
10、afftosubmitreportsonadverseeventsofmedication.4.2药剂科负责每年汇总、评价所报的用药错误、临界差错和药品不良反应,制定预防措施改进工作，修改不合理操作规程，并汇总给医务处和护理部。Thedeptofpharmacyshallcollectandanalyzethereportedmedicationerrors,nearmissesandadversedrugreactionsonyearlybasis,workoutpreventivemeasures,modifyirrationalproceduresandsubmittheimprovementtodepartmentsofmedicaladministrationandnursing.4.3医务科和护理部将本院发生的药品不良事件通报临床医师和护士。Departmentsofmedicaladministrationandnursingshallcirculatetheadversemedicationeventsincliniciansandnurses.获经批准Approvedby院长Hospitaldirector日期Date
