2、施工组织设计1.2编制目的洞门凿除施工是盾构前期准备过程中的是极为重要的一个环节，存在着极大的危险性，一旦控制不好，不但会影响盾构的正常始发，还将对作业人员的人身安全造成威胁，并可能造成项目财产的巨大损失。为充分利用项目资源，合理规划运作方法，以处理在洞门凿除施工作业过程中可能发生的各种紧急情况，实施在发生重大生产安全事故时的救援和事后处理工作，尽量减少事故的危害和影响，保障项目部施工人员、周边居民和可能受事故影响人员的人身健康和安全，保障项目部、周边单位和公众财产的安全，特制定本预案。1.3适用范围本应急救援预案仅适用于本项目右线盾构始发洞门凿除过程中可能发生的各类险情及安全事故的应急救援及

3、处置工作。各部门及相关人员将依据预案要求，对相关措施组织学习。二、 工程简介本工程范围为深圳地铁一号线（罗宝线）续建工程土建20标段固戍站后瑞站地下段土建工程右线盾构区间。固戍站后瑞站区间起止里程为SK35+037.20SK36+800.00，右线盾构隧道总长1762.163m（短链0.637m）。区间线路位于深圳市宝安区宝安大道下方，线路南北走向，盾构机从明挖段南端（里程SK36+800.00）下井始发，沿线穿行宝安大道下方，中间通过下锚段明挖施工段（里程范围SK36+000.000SK36+061.600,总长61.6m），到达固戍站北端头（里程35+037.20）解体吊出，完成掘进施工。

4、管片外径6000mm，内径5400mm，管片厚度300mm，管片宽度1500mm，混凝土强度C50，管片拼装形式为错缝拼装。三、 施工作业过程中的危险性分析1、 作业人员作业过程中未佩戴好安全防护用品，造成伤害事故；2、 洞门凿除作业人员未持证上岗，在接电及钢筋切割过程中，违章操作引发事故；3、 洞门凿除搭设作业平台不稳，作业时坍塌造成作业人员伤害事故；4、 作业前未进行专项交底及安全技术交底，作业过程中不按照方案施工、野蛮施工，造成事故；5、 洞门凿除过程中动火作业及明火作业未办理动火审批，防护措施不到位，造成火灾事故；6、 洞门凿除过程中交叉作业未作好防护措施，造成物体打击事故；7、 做检

5、测孔检测过程中，有泥沙或水流出，未及时采取措施，造成坍塌事故；8、 在剔除洞门上不稳定的水泥块时，有大块下滑，且有大块滑移迹象，未及时采取措施，造成坍塌事故；9、 空压机、风镐等设备进场不符合要求，使用过程中造成机械伤害；10、 现场物品摆放凌乱，渣土及废钢筋未及时清理，文明施工较差；11、 现场应急物资准备不充分，在发生紧急情况时未能及时有效采取措施，致使事故严重性增加；四、洞门凿除安全注意事项(1) 对各施工、操作人员进行安全意识及操作规程教育。各施工操作人员未经安全培训教育不得上岗，对特种作业必须持证上岗；(2) 现场施工人员一律要佩戴工作胸卡和安全帽，遵守现场的各项规章制度，非施工人员

6、一律不准擅自进入施工现场。(3)为确保凿除洞门期间安全，洞门凿除前，现场必须准备准备好砂袋、水泵、水管、方木、风炮等应急物质和工具，以便发生意外时可及时封闭作业面。(4) 开工前应检查工具、机械的性能。防止绳索脱扣、破断；钻孔桩凿除严格按施工方案的要求及顺序进行。(5)洞门凿除完毕后要及时清理作业面，避免有破碎的岩块塌落。(6)洞门凿除期间，测量人员加强对洞门及地面建筑物的监测，并及时反馈信息。(7)施工脚手架的搭设应牢固可靠，并应加设足够的支撑或拉杆以保证其稳定。高空作业应有可靠的安全保护措施（如：按规范要求系好安全带等等）；(8)做好用电安全教育，照明设备必须按规定配置有效的安全保证装置。

7、严格执行施工现场临时用电安全技术规范的有关规定；(9)做好施工现场的安全防火安全工作，现场气割作业要按有关规定办理相应的动火手续，并设置灭火器材以防万一。(10)在施工过程中，一旦与其他项目的施工（龙门吊吊装作业等）有冲突时，立即停止洞门凿除施工，并将施工人员撤离到安全区域；(11)施工现场防火、用电安全、施工机械管理及余泥外运、散体物料运输、使用预拌混凝土等应严格执行国家或地方的有关规范、规程和规定，绝对禁止违章行为。(12)垂直吊运土渣等物品时，要严格按照规范要求及技术交底；指挥人员应使用统一指挥信号，信号要鲜明、准确，吊机司机应听从指挥。(13)在施工过程中材料要严格定量使用，除合理耗损

8、外杜绝浪费现象，并采取处罚措施。(14)工地应严格遵守文明施工规定，并教育员工自觉遵守城市环保、环境卫生管理条例，做文明员工，共同为卫生城市作贡献。(15)必须采取各种措施，限制和降低施工过程中的噪音。五、项目部应急组织机构建立以项目经理何文锋为组长，其他项目管理人员为组员的应急救援小组，以便在发生事故后能及时采取措施进行现场救援，防止事故进一步扩大。 5.1应急救援小组组成及职责5.1.1组长：何文锋职责：负责应急救援工作的启动。负责事故应急行动期间各单位的运作协调，按照应急预案合理部署应急策略，和事故现场指挥者协同工作，保证事故应急救援工作的顺利完成。5.1.2副组长：曾德文、李明亮职责：

9、负责对事故现场的控制，协调应急队员的救援工作，识别危险物质及存在的潜在危险并对事故现场进行分析，执行有效的应急操作，保证应急行动队员的个人安全，并负责事故后的现场清除工作。5.1.3应急救援部门5.1.3.1 技术室陈俊毅，职责：担负应急事故发生后的技术处理，从技术方面提供处理意见；5.1.3.2 安监室王伟，职责：担负应急事故发生后的安全处理工作；5.1.3.3 材设室叶景雄，职责：担负应急事故发生后物资设备的供应工作；5.1.3.4 测量室温振波，职责：担负应急事故发生后的监测工作；5.1.3.5 综管室李毅昌，职责：担负紧急情况下受伤人员的初步救护工作；5.1.3.6 应急抢险队，由掘进

10、一班、掘进二班组成，职责：担负施工现场应急事故的处置任务。5.2、事故处理流程一旦事故发生，项目部要及时调动并合理利用应急资源，包括人力资源和物资资源投入行动；在事故现场，针对事故的具体情况选择先期处置方案，从而能及时有效地使伤害和损失降低到最低程度和最小范围。应急行动的优先原则：1、员工和应急救援人员的安全优先；2、防止事故扩散优先；3、保护环境优先。5.2.1、事故现场：（1）、上报事故现场、项目部负责人或安全员应采取以下行动：、掌握情况。不论现场何种局面，必须掌握的情况有：事故发生的时间与地点、种类和强度；已知的危害方向；事故现场伤亡情况，现场人员是否已安全撤离；是否还在进行抢险活动；有

11、无火灾与爆炸伴随；现场的方向、风速；事故危及项目外的可能性。、报告。在抢救、营救和撤离人员的同时，现场负责人必须将掌握情况迅速上报至值班经理或直接上报给项目经理。、分级。值班经理或项目经理接到报告后，对报告情况按照以下情况进行分级：一级：预警，最低应急级别，现场可以控制的异常事件或容易被控制的情况。二级：现场应急，属于中间应急级别，不会超出项目界限。外部财产一般不会受到事故影响。这种级别表明项目内人员已经不能或不能立即控制事故，需要外部援助（如消防，医疗单位的援助）。三级：全体应急，这是最重的紧急情况，表明事故已扩散到项目外。根据不同事故类型和外部人群可能受到影响，可决定进行安全避难或疏散。同
